You can find a copy of your contract in your Boulevard account. You can also call our front desk at 804.491.4111, and they can email it over as well.

Yes! There is a three-month commitment to start. After three months, it becomes a month-to-month contract.

There are a few ways to cancel or freeze your membership. Please note, you MUST speak to our front desk team in order to cancel or freeze your membership. Should you decide to fill out the form linked below, it is simply informing our front desk you would like to be contacted in order to cancel or freeze your membership.

All account balances and unused treatments or services will expire at the end of three (3) months, will be forfeited, and are non-transferable.
Refunds will not be given for unused treatments, services, or account balances at any time except for Death or Permanent Disability, Relocation, or Closure of Habit Med Spa and Facial Bar.
